Service Information;

This service lies outside the NHS and the post is overtly different to a typical NHS post in that there are no specified numbers of programmed activities (PAs) but is thought to be the equivalent of 12-13. At least 20% of the overall workload is private and is included in the total PAs as it is carried out in conjunction with Contract (equivalent to NHS work), in the same facilities and with support from the same PA and Administration staff. As well as the weekly timetabled SPA there are monthly academic half days during which all clinical commitments are cancelled, this amounts to another additional 0.5 SPA per week. There is also an expectation that the post holder will arrange an off-island link with a tertiary centre to maintain clinic skills and subspeciality expertise.

The remuneration reflects the workload, particularly that undertaken out-of-hours. There is no differentiation between contract and private patients’ clinics, and therefore some of the DCC sessions include private work, though consultants with a large private patient workload may choose to do extra out of hour’s clinics. Financial services regarding private patients are provided by the MSG

Below is an estimate of equivalence, with regard to direct patient contract (DPC) and supporting professional activity (SPA):

The indicative timetable below includes Duty Physician 1:10 daytime on call. This includes weekends and 1:10 nighttime on call, with prospective cover. After a night on call the following afternoon is free once the post take ward round is finished and if working a Friday/Saturday/Sunday days or nights on call, the Monday afternoon is free. Day time on call is from 8am until 6pm and night-time on call is from 6pm until 8am, followed by a ward round of your acute admissions. (Timetable in a separate document).

Admissions on call are relatively small with double figures being pretty unusual for a 24-hour period. Most of these arrive during the afternoon and early evening. Most admissions between 23h30 and 08h00 would be seen by the ED doctors and admitted for review by duty physician in the morning.

There are 35 days holiday a year, with the ability to carry 5 days forward into the next year if untaken. The Consultant is expected to work flexibly and to put the needs of the patient first. This is a Consultant delivered and not a Consultant led service, with no junior staff. The post is planned for daytime 10 PAs, 2 of which are SPAs. Private patient activities are integrated into this rota and can contribute at least an additional two PAs.
The on call is equivalent to 2-3 PAs
Direct clinical care sessions include: Administration work directly related to clinical care
Outpatient clinics
Ward rounds
Weekly day time on call activities
There is 1 afternoon off each week.
A mentor will be offered to the appointee at the time of their appointment.
